Picosecond Lasers for Stubborn Pigmented Spots
Age spots and sun spots are notoriously tough to get rid off. Just today, two people walked up to me pointing to a constellation of brown spots on their lateral cheeks, asking if Eocene has anything that could help. They were not your typical medspa customers – middle aged men who have never had a facial and who believe glabellar wrinkles are tokens of wisdom. But they are clearly bothered by these pigmented spots.
So what can we do to treat the stubborn pigments in dark age spots and sun spots?
My go-to right now is Picofy – the next generation, advanced picosecond laser form Inmode. Because the interesting thing about treating pigmentation is not a miracle wavelength. It’s pulse width: how briefly the laser delivers its energy.
What “picosecond” actually changes
A picosecond is a trillionth of a second. When a laser pulse is that short, pigment particles (natural melanin clusters in the case of age spots and sun spots) absorb the energy and break apart by a photoacoustic effect. Think tiny shockwave shatter the pigments, rather than cooking the surrounding tissue with prolonged heat which is how traditional thermal lasers work.
In plain language: shorter pulses can fragment pigment more efficiently while reducing unwanted thermal spill into nearby skin. That mechanism is one reason picosecond platforms became attractive for discrete pigmented spots.
It does not mean zero heat, zero downtime, or guaranteed clearance. Depth of the pigment, your skin type, and how faithfully you protect the area from UV still decide a lot of the story.
Picosecond laser for tough pigments
We often discuss picosecond technology when patients want help with:
- Stubborn freckle-like sun spots or other discrete pigment that has been evaluated clinically
- Selected textural concerns where carefully chosen settings may support remodeling over a series
The lesion must be confirmed or be benign (not melanoma or other types of skin cancer) before we can treat it with Picofy laser. Please check with a dermatologist if you have any lesion that is suspicious. Looks out for asymmetry, irregular borders, multiple shades of brown or black, or size larger than the eraser at the end of a pencil. Lesions that have grown or changed in shape and color within the last few months is alarming as well.
Picofy can be the modality to try if you’ve had sun spots that don’t seem to respond to brightening creams, peels, thermal lasers, or even resurfacing.
Pigments usually fade within 1 treatment, and clearance is possible, although it may require 2-3 repeated treatments. Once cleared, the pigmented lesions will not reappear. But you may see new spots appear in other areas of the skin, as the skin will still age and responds to UV light exposure. That’s why regular sun protection is important, to both prevent new skin damage, and guard the hard work you did to correct skin pigmentation.
Melasma is a different diagnosis, but we can still help
If the blotches shift with hormones, pregnancy history, or summer sun / warmth, we may be talking about melasma.
Melasma is a chronic, relapsing condition. Traditional thermal lasers are not suitable for melasma because the heat energy will make the lesions worse.
Picosecond lasers are relatively safe for melasma. Careful and incremental adjustment in energy settings guided by medical knowledge is very important. It’s definitely not a casual zap.
Picofy treatments can improve the appearance of melasma and fade the pigmentation. The areas affected do not “erase forever,” and unprotected UV commonly brings the pigment back.
