“I’ve tried everything for my acne scars.” I hear this a lot in consult. Topical retinoids, chemical peels, serums ordered off Instagram — some helped a little, most didn’t move the needle on those textural divots that catch the light the wrong way. If that sounds familiar, let’s talk about radio-frequency (RF) microneedling for acne scars. In this installment of “What’s the Evidence” series, we’ll turn our attention to three studies published in 2026.

How RF microneedling works on scars

Standard microneedling – an array of tiny needles that prick into the skin – creates controlled micro-injuries that prompt your body to lay down fresh collagen. RF microneedling takes that a step further: insulated needles deliver radiofrequency (heat) energy directly into the dermis, generating precise thermal zones beneath the surface without damaging superficial skin cells. That controlled heat activates fibroblasts — the cells responsible for collagen and elastin production — and triggers a remodeling process that gradually fills and smooths scars from the inside out.

The key word is gradually. This is not an overnight fix. Collagen remodeling unfolds over weeks to months, which is why treatments are typically spaced 4-6 weeks apart in a series of 3-4 sessions.

How does RF microneedling compare to skin resurfacing?

If you asked me how to treat scars 15 years ago. I’d have said, ablative skin resurficing with a CO2 laser. In my training at Massachusetts General Hospital and Shriner’s Hospital Boston, we actuially treated a lot of scars this way. The CO2 laser evaporates the scar tissue, giving the skin a second chance to grow back smoother. The process was bloody, and painful, requiring general anesthesia.

CO2 skin resurfacing has been around for so long that it’s the benchmark every other treatment modalities is compared against. How does RF microneedling measure up?

A prospective, randomized, split-face trial published this year in the Journal of Dermatological Treatment compared RF microneedling head-to-head with fractional CO2 laser in 30 patients with acne scars. Each patient had one side of the face treated with RF microneedling and the other with fractional CO2. They all received three sessions.

The results: both treatments produced significant scar improvement. Scar severity scores dropped by roughly 44 percent on both sides, with no meaningful difference in effectiveness between the two modalities.

Where RF microneedling pulled ahead was in recovery. Patients on the RF microneedling side experienced shorter durations of redness, swelling, pain, and scabbing. They also had fewer episodes of post-inflammatory hyperpigmentation – darkening of the skin after inflammation. This suggest RF microneedling could be the better choice for patients with medium-to-deeper skin tones, where PIH can be as frustrating as the scars themselves.

How strong is the evidence?

A separate 2026 systematic review examining 41 studies across diverse Fitzpatrick skin types confirmed the pattern: RF microneedling consistently reduces scars. The effectiveness is similar to ablative skin resurfacing, and RF has high patient satisfaction and minimal downtime.

RF microneedling also has the added benefit of reducing skin laxity and photoaging. It improves wrinkles, builds dermal collagen density, and reduces submental volume (double chin).

Who benefits most

RF microneedling tends to work well for rolling and boxcar-type atrophic scars — the broad, shallow depressions and sharp-edged divots that make up the majority of acne scarring. Deep ice-pick scars may need a different approach or combination strategy. Combination therapy with PLLA (poly-L-lactic acid, a biostimulant and promotes collagen production, brand name Sculptra) and exosomes with RF microneedling can produce a synergistic effect that tackles even the tough scars.
